It's been a couple years since I played it, but I vaguely remember it was some sort of artificial virus? I mean, they're a terrorist group in a post-terrorism world. That's the whole point. Harman is good, and Kun Lan is evil, that's why they're still alive 100 years later.
Like I said, read Hand in Killer7. Most of the "lore" is explained there, but if you want to save yourself the headache just take it at surface level. A lot happened in that game and the story is not "unfinished"--everything is told to you, just not explicitly. Play it again! It's very interesting a second time through. One of my favourite games ever.

The game's plot is easy enough to understand, but the somewhat disconnected and strange nature of the levels and plot make it more 'complicated' than necessary.
It's like a math related word problem; you're given a bunch of stuff that isn't all that relevant to the point at hand, which can cause overthinking when the actually necessary pieces make things simpler.

The Smiles are humans who are, apparently, under Kun Lan's influence. He touches them with his shining fingers, turning them into living bombs. He then sends them out to go blow up places/people in order to spread fear.
Why? I don't know. The game doesn't go into his reasons except that he's (probably) playing some sort of human chess game with Harman.

Did you get all the pigeon messages? Those reveal a lot. Take your time and really pay attention to every cutscene now that you know the truth. It will come together, and what doesn't can be imparted through Hand in Killer7.
