The achievement checklist.
There are a few you can miss every time if you do it wrong, like missing rosaria's soul or the silvercat ring, but none of the + rings.

They are easy to beat.
But if you want to get all the stuff, it's pretty much impossible.
Many sidequests involve backtracking at specific times to areas you have no other reason to revisit. No amount of playthroughs will tell you how to complete that sidequest. And if you go to far? Gone.
If you never summon, you can't get all gestures. I didn't even know npc summons gave gestures until I finished my first playthrough.
Then there's all the little nooks and crannies and sidepaths within sidepaths within sidepaths that hold items that you otherwise wouldn't know existed without a guide. Like, cursebite ring doesn't have a +1 version like all the other bite rings. You could scour for hours looking for it.

I play offline and summon NPCs for bosses so I figured out early you get gestures from summons.
"sidequests" as you call them are not terribly obscure. Sirris was weird, but when you're high SL and you're goofing around co-opping bosses you notice it's really strange there's a summon sign on the ground after you've beaten an area.
As far as the rings go yeah I didn't get all the rings until NG+6. Thankfully you can get the normal, +1, and +2 versions when you're in NG+2 or beyond.
