You go to the end of the level. There's a ring somewhere that locks your HP at 75%.

This, also use firebombs on the boss, so get as many as you can find, also use turpentine on it. The slug things are weak to fire.
The "boss" is really just all those slugs with shields, once you kill all those the boss has no attacks and you can kill it without any trouble.

Assuming you died to Phalanx then you just need to teleport back to Bolivaria from the nexus and run straight forward through the gate you opened.
Also you have half health because you're in "soul" form, get your body back if you want full health.
