It was just a case of lots of problems that added up into a fucking annoyance.
Alchemy was pretty dumb. Having to go into that meditation menu each time you want to pop a potion was annoying and it also meant there were big fights in the game you couldn't actually use potions for because they'd run out and you wouldn't be able to use them again. Alchemy ingredients having as much weight as they did was stupid considering how you have to keep crafting the same stuff over and over again.
For combat the riposte system was silly and for the most part it was just spamming roll/quen, I think W3 gives a bit more variety on that front.
Other than that it's minor annoyances for me. Minigames weren't very good, menus were awful and I thought the boss fights were really fucking stupid.

Doesn´t really shine in terms of gameplay but the content that it has, the music , the graphics , the setting and its attention to detail makes up for it.

Spamming quen became a shit strategy when they nerfed it. This meant that the best strategy was spamming roll/roll, although my favorite strategy was spam reusable generic traps.
I found that Witcher 3 refined pretty much all of 2's mechanics.
