It should have had a 25 minute tutorial section where it explains all the details of movement and stuff. Also giving players a second chance per hit would be nice plus the boss sections should have let you build turrets and shields and stuff to make them more accessible. Some MMO elements wouldn't have gone amiss either so you could voicechat your friends if you were getting stuck.

They should have based the game around emotional skill instead of mechanical skill. When you bump into the first Goomba who's pretty much unavoidable to all but obsessive gamers you should have a dialogue tree.

Also, the fact that the main power up is a phallic symbol is very problematic, a vegan taco would be a better powerup since it resembles a vagina and vaginas have power and a voice, the option to play as a female or trans character would be nice too since roleplaying as a cis italian macho is hard for most gamers.

The NES cart insertion method was so stupid. There has to be some way to make the entire mechanic more interesting and not so obvious when it can be used.
SMB1 is very well designed. All I could really think to do which I would consider as justifiable would be ways to make score mechanics more interesting and rewarding.

It's a thing coming at you, and since you probably already pushed a button and found out you can jump, you're naturally gonna assume it's an obstacle. Especially in those days when there were no friendly NPCs or anything in games so everything was an obstacle or hazard.
Worst case scenario, the player runs into it and learns what happens when you touch the brown nutsacks and has to go back ten seconds to the start of the level.
With the Super Mushroom, it's specifically designed to bounce off a ledge so as to come towards you, in a spot you're unlikely to be able to dodge it since you can't backtrack. It's meant to be almost guaranteed to hit you so you'll see that it's an item and you want to get it because the little noise it made released sweet, sweet dopamine into your brain.
The entire concept of the game is demonstrated to you in these maybe 20 seconds of gameplay: jump over stuff, hit blocks, get items, keep moving right.
