I'd say it's stuff that makes you scared for a reason besides a survival instinct. It can be something extremely esoteric and metaphysical like fear of death, or fear of the cosmic unknown. Or it can be a pointless fear that seems to only cripple you, like fear of public speaking, fear of losing your mind, or just a fear of your own existence.
What these things share is that the fear is pointless, in that it's not a sensible reaction to the environment, but a festering boil on your psyche.

And if you define it like this it's pretty obvious that games can't get it right. Games are always about reacting to the environment and solving problems. They can only really reference these fears, unless the game is actually more like a book or story than a game.

No. Games have done certain aspects. But nothing that completely destroys a man's sanity.
Feeling of edge, the lingering panic, nightmarish after shocks. Untrustworthy feeling and anxiety. And so on.
Best I say penumbra, Eternal Darkness, silent hill and a bit of RE. Only have certain parts that would mesh to create the umbrella term.

Silent Hill is the most obvious example, but there's a few games that aren't horror games that have done psychological mind-fuckery well enough on their own.
I remember being pretty scared of some of the environments in the old Tomb Raiders, simply just because of the alien-like nature to it, the massive "What if..." that came to mind whenever the game did its tricks. I remember swimming across a cave once, and the game panned the camera out above Laura to show how fucking deep the water went below her.
The Cradle from Thief is also just as good a fucking example, as well as the one mission from SWAT 4 that took place in that haunted as fuck drug house or whatever.
There's a lot of games that have done psychological horror right, but they don't capitalize on it because that's entirely the point. It makes it that much more unnerving. There's a lot of ways that Silent Hill doesn't capitalize on the subtle shit that could freak you out right proper, such as howif a room has a mirror in it in Silent Hill 3, there's a camera angle positioned on it, or at an angle where it can't be seenor evenhow James jumps down hole after hole into the sewers, going well deep enough underground, yet exiting to the surface as if he was up there all along
It's the little things that really get you. The only thing keeping you away from really understanding it is the subtle rejection of it from your psyche.
