Mortal Kombat 3, maybe? That's the only one I remember that actually had an ESRB "M" rating. Other stuff used internal rating structures, like Sega's VRC, or that temperature gauge thing for PC games.

My parents were clueless about ratings and believed that all video games were for kids. Most of the games were borrowed from my friends or shareware versions so they never saw a box with M rating and were ok with me playing pretty much everything. Wolfenstein, Doom 1 and 2, Turok, Blood, Quake 1 and 2, DN3D.

When it was new, I really wanted to play RE Code Veronica from the video store. My parents took a lot of convincing that I was mature enough to play an M rated game, but they eventually let me rent it from the local blockbuster.
In the end I really sucked at the game, and couldn't make it past the first group of enemies. I got frustrated and quit and was mad that I wasted my rental and argued with my parents for nothing.
