I never take the default name because if that name was important they wouldn't let you change it.

In pokemon I use my own.
In games where I get to customize my guy I make a character up and invent a name that suits him.

Depends on the game. In Pokemon and shit like Zelda I go default. I couldn't even enter my name into a Pokemon game because you only get like 7 spaces and my name is longer.
In JRPG's and games where you can heavily customize I go with my own.

If I get to customize the character, I'll usually do myself, or a random theme I think will be fun for the game, usually if I've played it before.
If I don't customize the character, I keep it default.
