The thing about horrors games is you're expecting to get scared.
The biggest scare I've ever had in a game was where I was least expecting it. Playing motherfucking Wolfenstein Enemy Territory. I was sneaking around in a part of the map where I was confident that nobody else would be. I turned around and there's this fucking Axis engineer standing right in front of my face, out of nowhere. I nearly fell out of my seat.

What really scares me the most is just when you know a game inside and out and something out of the ordinary, or 'not supposed to be there' happens.
For example, I know like every fucking spawn in FFXIV: A Realm Reborn like the back of my hand. Nothing really ever surprised me in that game. Eventually, they put in a patch that added 'hunt' monsters, rare spawns that would pop up once some weird condition would be achieved. At first, nobody really knew what these conditions were so the rare S-Rank Spawns weren't documented for shit.
One especially dark in-game night, I was running around gathering things in Coerthas, when all of a sudden I turn around and 50 yards away from me is pic related, in the shade, eyes glowing fucking red, staring right at me. This picture doesnt do it justice, but it's huge, and in the shadows it was barely visible save for its eyes. It looked EXTREMELY out of place and I let out an actual yelp IRL because it surprised me so much. Shit was cool and it has stuck in my mind since then.
Otherwise, just strange glitches and shit that should not happen. You ever have an old cartridge get jarred/fucked up in the system, so the picture turns into digital garbage and it starts making some high-pitched, awful screech? Or maybe just the sound/video becomes fucked up and distorted? That's what gets me the most.

Water's always a fear factor for me, even in games where I know there's nothing there, where players have dug up the code, studied it to hell and back and confirmed there's nothing there. Even in games like motherfucking WoW I'm scared to go into water, specially after my first encounter with the Whale Shark cadaver in Thousand Needles. Another game that comes to mind is SotC, though Hydrus's battle doesn't seem scary for some reason, I always freak out in sections like the swimming part to reach third colossus and shit.
