I think it's 95% word of mouth about its difficulty. It's a supply and demand thing. The vast majority of AAA games today coddle you so just being one of the very few alternatives to that is a selling point to a good number of people.
Having a good style to all the creatures and equipment certainly doesn't hurt either. I think most of the people who are into it want a difficult game with good graphical fidelity.

The majority of a typical AAA game's budget will go to marketing. I don't think that's really a fair way to break it down since I'd wager the marketing for Dark Souls has been a fair bit lighter than a typical AAA American game.

I thought LoTF was, well it was the best "ok" game iv played in awhile. Id did a lot right though, the designs were great except for the enemies who all looked like sickdark diablo shit, and the BEEFY MCLARGEHUEG protagonist. However the world design and architecture was excellent.
