Cheat codes born for the simple fact that a developer needs to try/explore ALL the game, for find some bugs or glitch in the game.
For example:
The original pacman crashes at level 256
How did they find?
in the code of the game they program a code that allows them to go to the next level with no effort, so they can see if there is some kind of bug.

best way for dev to implement cheat codes is to allow user access to dev-console.
Unreal Engine games, Source engine games and Elder Scrolls series come to mind as having implemented that the right way.
Console games and PC ports of consolefag games usually don't have that option (due to no keyboard on consoles) and because of that the dev-console functionality can be packaged as DLC and used to milk consolefags

Partially this and partially in 2005 Xbox 360 came out. Because it relied so much on Xbox live, you couldn't very well add a debug mode or cheats to a game that had achievements to keep track of.
This is why GTA IV will disable your save or achievements when inputting a code.
That whole new generation of systems also killed modifiers like gameshark and action replay.
