For me it's usually how tightly I'm pursing my lips, so how much air is able to come through. The tighter they are the higher the note and vice versa.
I've also been fiddling around with how I place my tongue in my mouth, which has made some higher notes easier, for myself. I'm decent with mid and low notes, but high ones are my bane.

I couldn't whistle for the longest time. A buddy in one of my Italian classes told me to practice getting a decent sound by pursing my lips and blowing out then sucking in. It was a good way to start.

It's something that, for me at least, come from the back of the throat. You can also do it by quickly changing the tightness of your lips, but that makes it sound a little muddier.
