Just got the last ending, this game was fucking great, but I have to say I was expecting more from the ending.
Can anyone explain to me what the fuck was going on in the story? I don't understandWho was doing these experiments on the people to make them shades? Was the disease natural or manmade? Which is the true ending, and when does the sequel take place?

The Grimoire Nier should answer most of your question. Pretty sure the sequel takes place centuries later after robots start worshipping Emil and killing off humans or something so now they live on the moon and you play as an android sent down to see if everything's still robo-murder-rampage down there. I could be a little off though.

tl;dr: Drakengard ending E caused the disease, it was neither natural nor manmade, but caused by the death of an otherworldly magical being. The experiments were done in order to save humanity, as they'd die out otherwise. We're not sure which ending is the true ending, the sequel might tell us. And the sequel takes place an undisclosed period of time after the first one.

Emil is confirmed to be in Automata, while Nier and Kaine will probably be mentioned. This is coming from Yoko Taro.
