I'm running unique bodies for followers right now since they'd never get gib'd anyway. I install CBBE, run bodyslide until I have what I like, go to the folders where the body bits are and drop them in the unique follower folder, then uninstall CBBE but leave on the CBBE compatible vanilla clothing. And voila! Tits on my follower but normal bodies on everyone else and gore works.

It's actually lower than most of modern Bethesda's games. Off the top of my head the only ones unkillable are all children and the core Minutemen squad, since they're the default "Yes Man" guaranteed victory choice. I know you can krump the other factions before you even get to their points in the story, mostly.
