In universe its just as bad comparatively as in the real world.
The police are corrupt, entire districts have just been given to the gangs because there is no chance of recovery. If you go on shooting sprees in GTA IV you can sometimes hear people say things like "I thought this was Liberty City, not Carcer!"

For a new city, I reckon a Boston alternative would be sick. The old, established, Irish mob is forced to confront a far more brutal invading gang of hispanics in an all out turf war. On the side, the corrupt police are playing both sides to stay in their good graces until a winner is chosen. Play as characters from each faction and engage in missions (like cops responding to gang shootouts) where you can choose to play a particular side to benefit them, and even terminate your other playable characters if you want.

Look, I'm not even American and I can identify Boston just from its older buildings and sprawl. It's not got anything as individually distinct, but it's a fairly easy to guess the location... as if the Irish mafia wasn't enough.
GTA will always be the US from now on, so let's stop throwing around other countries. They've made it clear the satire of the states is what they're most comfortable working with.
If I had to guess, it might be VC or LC again. If they wanted to move away from the big three, maybe Washington, Texas, Louisiana, Mississippi, Carolina, Alabama or Carolina. These places are quite easy to poke fun at, and they're next to the coast. This makes it easy for Rockstar to create their invisible wall/island map.
