Play any source game and use noclip to get out of bounds. Once you pass the skybox it's just going through nothing but blackness that's not registered. In Pokemon Diamond/Pearl I remember walking out of the map with Action Replay and it just being registered as an unknown zone and playing generic route music.

You didn't even need to use AR to get out of bounds in D/P, you could actually ride the bike back and forth over a loading zone fast enough to break the game and go out of bounds. If you saved and loaded in the right place out in the void, you could appear anywhere on the map. But if you did it somewhere that was still out of bounds you would destroy your save file.
But if you knew exactly where to go (with the step counter) you could get to the event pokemon areas and stuff.
