The Joyful is more of an epilogue than a flat out sequel, you can finish it in a few hours. It's pretty divisive in terms of it's plot, and you get to control a couple characters in battle (instead of over two dozen characters in painful) so the gameplay itself isn't as good as Painful.

You know, some rpg tropes come directly from old ass rpgs, games made to be challenging and hard on purpose. Developers used those tropes, like the ones you listed, to make the game last longer and to offer some kind of challenge to the player. Of course I am talking about gen 3 and 4 games.
Many modern rpgs refuse to leave these tropes behind, for many reasons, like trying not to alienate old rpg fans.
My guess is that since rpgs need a story more than any other genre, devs focused more on plotlines and characters rather than made the rpgs' gameplay evolve with the times.
