Im still going through my first runthrough, the game is so goddamn huge and it seems like there are 10 sidequests for every area of the map that a main quest leads you to, im in velen at level 10 and my main quest is still at recommended 6 because Ive been running around doing bullshit for 30 hours because theres so much shit to do, especially on an alchemy build.
You can literally go crazy with this game and play 20 hours of sidequests before the next main and you wont have even made a dent in the map.
Few comments though:
1)foglets are fucking hell. Can never dodge those fuckers because their second swing lunges forward and catches you out of the dodge
2)I hate running into high level fuckers I have no chance of beating. I mean, im riding down the main road at a sub-10 level and a gang of level 20 deserters decides to fuck my day up. You constantly run into shit that is out of your league, the game encourages exploration and then punishes you for getting out of the main quest line. Nothing quite like hacking at a level 25 regular mob boss in a cave and having to hit him 3000 times with alchemy support because there was nothing to tell you that a boss 10 levels above his minions is at the end of your journey. The buffed up normal enemies are the worst; they have the attack power of a main boss while having the speed of a mob enemy.(that fucking foglet that has a 2000 damage fast attack against my 4400 vitality character. Fun times.)
3)the horse. Holy fucking shit mang is this thing buggy and unresponsive. Falling off your horse from fear because he hit a small rock in his way that brought him to a complete stop while surrounded by enemies. Too bad it takes 3 seconds to get going again or I might have still got away. Too bad, guess Im fighting a level 5000 wolf now with 5000 attack. Fun times.

It asks you to go into the stables behind the dude's tent and find a pickaxe. And I know exactly where it is, it's leaning up against the wall in the corner of the stables next to a barrel. Except I don't get a prompt to pick it up, and Witcher vision doesn't even highlight it that it's interactable.
