In the ones released in English it's mostly the latter. You can get through most of the story with the same monsters if you put a good bit of time into levelling, though it'll only get you so far.
In the most recent ones, released only in Japan, though, you can improve any monster species by synthesizing it enough times.
How it works is that basically, each time you synthesize a monster, it gets +1 added to its rank. At +25, it becomes C rank if it was lower than that. At +50 it becomes A rank if it was lower than that, and at +100 it becomes SS rank, the highest rank in the game.
There are shortcuts to increase in +s faster, so it won't necessarily always require 100 synthesizes.
