Nobody, because there are very few programmers as good as him left. IDTech2 was a 2D engine fampai, but it looks 3D. That's pretty impressive, but it had to be done otherwise it wouldn't run on a 486.

Ultima Underworld had a full 3D engine and that ran on a 386 a year before Doom existed. Carmack isn't as good as you think he is.

Not him but didn't Ultima Underworld (going by what I played of the original System Shock, which is on more or less the same engine) still use sprites for almost everything? The game world was maybe 3D but pretty much everything else was 2D. Which is still impressive considering Doom was completely 2D.

Doom's optimizations came from a research paper written about back-to-front BSP culling that Carmack happened upon. He didn't even come up with the solution for it.

Do you mean programming wise? Maybe Johnathan Blow, mainly because people actually know who he is. He's obviously very passionate at programming, cranks code out quality code very quickly and is working on that language in his spare time. There's lots of good programmers out there, its just that the ones that are half as good as Carmack are completely unknown.

Programmer + co-creator of a revolutionary game or genre? Doesn't exist.
