So... why didn't it just come to Riku immediately? They Keyblades have their own delivery service? Why didn't it just stay with Riku when he grabbed it in Twilight Town? It just feels like Nomura came up with a plot twist for the sake of a plot twist and didn't really care if it made sense or not. At least it's one of the most egregious parts of the plot in the first game, that's nothing compared to the horrors of the later games.

It's simple if you really think about it.
When a heart is lost and becomes a heartless, it's a metaphysical fracturing rather than a true death. The heartless does its thing, while the body and soul are left in a sort of limbo.
If the heart was really strong, then the imprint left on the body and soul allows them to break out of this limbo and become a fake being that just has the memories of the original person. A nobody. When a nobody "dies", the body and soul regress back to that limbo. The nobody was never truly alive in the first place; you can't kill one any more than you could kill a rock.
So, what happens when a heartless is purified by the keyblade and the body and soul get sent back to their stasis? The three merge and the original person is reborn. That's what happened with Axel and the others.They're lucky, because apparently this process can go wrong. If the heartless isn't taken out before the nobody, then the comeback isn't guaranteed.

Blank Points pretty much shows that it's Xehanort in control and he's been fighting Terra over his body constantly and Terra's clearly been losing. The memory thing in general is up in the air. But as we know right now, in the case of Terranort/Xemnas/Ansem SoD, they're all predominately Xehanort in nature/mind. Though Terra's memories have been confirmed to be present in Xemnas.
