The main quest line story itself doesn't carry enough weight on it's own, so you're forced to do a bunch of side shit and expected to be impressed by the side quests.
Only thing is most of the side quests are repetitive and the rewards are awful, there's like 5 quests in the entire game that are kinda interesting but have jack shit to do with the main storyline.
omg guys we have to go find Ciri NOW!.... but first lets do all this menial shit that doesn't matter in the grand scheme of things.

That's not the only way to get quests, the question marks generally aren't quests. You get them from notice boards and townfolk. That's the point of turning the points of interest off. So while on a witcher contract you might stumble across said spy instead of seeking him out.
