UnReal World, it's a low fantasy iron age roguelike set in a not-Finland where you will freeze to death in winter and hunting is actually pretty difficult and exciting. Magic is just rituals where you pray to the gods and hope they will help.

Morrowind is pretty weird fantasy set in a non-European part of a generic medieval European setting, though it's pretty easy to just regard it as generic medieval fantasy with a fancy coat of paint.
I guess you could call Sword of the Samurai an rpg? A sort of rudimentary RPG/sim/strategy mash up. It's a fun time killer, regardless. Also depends on whether you regard feudal Japan as generic medieval.Undertale.
I don't know if you'd call Dishonored an RPG. It's way more of an action game but I believe your choices have some impact on the game. It's Victorian/early 1900s with magic and advanced technology powered by whale oil.
