The Mensis Ritual opened a portal into a realm (Nightmare of Mensis) where Mergo is still alive, being cared by his nurse. The ritual is what causes the moon to become red. It blurs the lines between Yharnam and the Great Ones, allowing humans to be able to make contact with them. The brain was created by Mergo when it didnt understand what the Mensis people meant by "give us eyes in our heads". They had no idea what to do with it, so they just used it as a watchdog for the nightmare.
I think Rom is trying to stop the ritual because she sees contact as a negative thing and knows that one more bad night will cause all of Yharnam to collapse.

The most efficient BLT build can utilize pure BLT with the Chikage/Bloodletter, but it's a pain in the ass to get that far on just BLT so most people opt to go STR/BLT or SKL/BLT.
