I don't care about achievements that much but it bothers me not unlocking and achievement that should be unlocked just because I was playing offline and not being able to unlock it anyway after because it's bugged, that's why I never touched Dead Rising 3 again (probably also because it was boring). Also cloud fucks up your saves even when you're playing on the same PC, if you play a game offline and then save it there is a rare chance of cloud saving overwriting that save when Steam is back online and you lose all your progress, it happened to me a few times.

In my experience, playing in offline mode doesn't affect the total playtime that you see in your library when you're online. I can only assume that Valve anticipated that people might cheat the refund system by playing offline, and it's possible that they came up with a way to prevent people from doing this.
