If you see the boss battle between the two then you'll notice that Big Boss is just poking out for a few seconds at a time and even then Snake isn't looking at him because he is shooting at him. Also when Venom dies Snake probably hit him with a rocket launcher and his corpse was probably unrecognizable.

The horn was already pushing out of his head, yes in the flash forward during The Truth he has it, but he's also covered in blood - it's symbolism. The actual horn was removed.
