tf2 uses a really old version of source and its coded like tangled spaghetti (also why valve won't even bother porting it to source2) This combined with the seriously flawed lag compensation and the way hitboxes and collision hulls work make lag compensation work always in favor of the attacker, especially when he has a bad connection.

The Souls series handles backstabbing even worse, and pvp would be much much more intriguing if they just removed bs from pvp entirely.

I love the concept of backstabbing. You're rewarded for sneaking up on somebody with a one-hit kill and even if you didn't have an insta-kill backstab mechanic you'd likely kill that person anyways when you catch them from behind with their pants down. Like a Heavy that you didn't expect face-to-face from around the corner who mows you down with his minigun.

Dark Souls 1 backstabbing is pretty fucked up. I've had really bad experiences with it where a guy is walking directly at me as I'm backpedaling away and his character teleports behind me and does the backstab. The netcode in that game is horrendous.

I like how d&d and most tabletop systems handle it.
Extra damage, but not necessarily an instant kill. That way the fight is still challenging and you can handwave it as a constant state of awareness that allows your enemy to move slightly away from the blow preventing you from oneshotting them.
