SH1 is nearly the perfect game.

>not strong at combat
>protagonist cannot shoot a mite off a fly at fifty paces
I would add a stamina meter so that you can't run forever, can't swing forever, etc. I would also try to do some "director"-tier shit a la L4D rather than fixed monster spawns. I'd like it to also have a more open environment; I don't mean open world necessarily I mean that not so many locked doors overall. SH1 had the most exploration and there still wasn't much.

Get rid of all the humour. Add more shit to the existing gameplay, overhaul the SFX.
There done. Go do missions for the batshit crazy locals in the Horror Fucktown while figuring out whatever whichever cult is doing what-for in Horror Fucktown.
It'd be a spinoff probably, I don't think any SH fan actually wants that type of game.

I'd like to see a silent hill with multiple characters all trying to solve a piece of the puzzle at the core of the game with each having their own demons to fight. The demons would take the form of whatever was haunting people. They could feel guilt for killing/abusing someone or ruining someone's life or not having the courage to do something. Not all the characters make it to the end but all of them serve a purpose in the general context. Some of them even have to team up for certain parts of the game.
It could be a multi-perspective trip through what haunts and frightens people and how they deal with it or run from it.

I had an idea similar to this too. It had a mechanic, a paranormal investigator, a college student, a mother looking for her daughter, a man grieving over the death of his wife and daughter, a registered sex offender, and guy who is revealed at the end to be a wondering, tortured spirit of the town.
