No, you get a whip and bombs. You can kill a shopkeeper to get a shotgun, but that effectively puts the game into hard mode. Most of the good items are movement related, like the jetpack and cape.

It's only hard until you get to the ice caves, then all the shopkeepers start killing themselves. If you've done your black market looting, you're also well-equipped for the rest of the game.

When Spelunky was first released on XBLA I put in 100+ hours, I was addicted. I'm probably one of the few couple hundred people who have the speedlunky achievement (finish the game in under 8 minutes.) and I was ranked 48 on the leaderboard for fastest times.
That was 2, or 3(?) years ago now. Haven't touched my xbox since then, to be honest. Now I have it on steam and only put an hour into it.
I was probably bumped up to rank 150 or so since then.
Fun game.
