I always go big. I'm tall irl and I just can't imagine myself as small. Even when I make pretty ladies in the character creator they are all above medium.

Small usually.
When I play physically weak characters that succeed with dexterity or magic I generally like to play them straight.
When I play tanks (my favorite archetype) I like the juxtaposition that also supports the idea that they have a lower center of gravity that they can use to maintain a stable grounding.
I don't usually play the barbarian type that's a front line fighter focused on dealing damage with strength. I suppose that's the only time I'd choose large since reach is so much more important in that situation.
small, because the animation speed scales so it looks like you're speedy gonzalez. whereas bigger characters give the illusion that you are crawling along at a snail's pace because of their exaggerated animations
even though it's a cosmetic effect it's still annoying to feel slow
