If you want graphics above everything else then Ark: Survival Evolved is your best bet.
It's definitely the most graphically impressive game available at the moment, though plagued with optimisation issues. Its gameplay leaves a lot to be desired though.

It renders the entire map you're on though compared to Crysis 3 which renders a very small map (I don't know how much it renders on that small map though). MGSV also renders the entire map, but trades quality for performance by pretty much only rendering bases when Snake is near.

The Order looks good but a little too refined. There's barely any clutter, surfaces are very smooth/reflective and lack a layer of grime, nothing really stands out in the monochrome images, etc. It's like the second pass of a level before you put all the detail in.

People seem to think that realistic games don't have any art direction just because they aren't heavily stylized. There isn't this binary "Realism vs Art" that people like to think there is. True realism would just be google maps or something.

The art direction in a realistic game is just more in line with that of live action films.

Crysis 3 renders 4k textures, the shadows of every blade of grass, as well as water, grass, and object physics.
It has every right to be demanding.

Those open world games render a small area surrounding the player in low quality compared to before the downgrades, and nothing else.
In the cities the framerate goes to shit no matter what and to this day still has horrible popin.

Game also can handle itself at 4K on 980ti and FuryX.

4k textures aren't demanding though if you have enough VRAM and if the game uses that VRAM efficiently. Crysis 3 was pretty well optimized and I don't remember the texture quality having a huge effect on my performance.
The lighting and overall effects though are great and the game does have more advanced physics than the Witcher 3.
Witcher 3 pre-downgrade had the exact same textures and models though. The only differences are the lighting, the LOD, and various other effects. There's an LOD mod in the works that should fix most of the issues there and one of the main lighting mods I use is close to release with some actual improvements. The rendering hasn't changed at all from pre-release to release though.
I don't know what issues you're having with cities though, I only get like a 5 fps drop with my shadow tweaks and LOD mod.

Far Cry 4 looked so fucking good, but the game was completely fucked on the north end of the map. Nvidia Soft Shadows literally made the entire north part of the map flicker and that was my favorite part of the game minus the amazing effect that trees had that I forget.

It's not really a fix though, it's just forcing the game to load higher quality meshes already in the game. It lowers some performance and introduces some really weird pop-in, which is why I assume CDPR chose to modify the LOD and make it the way it is.

It's not a tiny bit of pop-in, it's more noticeable than whatever pop-in is already in the game.
The blue fog wasn't even to cover the LOD issues because the distant detail of trees and terrain isn't really bad. If you watch the Sword of Destiny trailer the LOD for distant buildings is the only thing that's been downgraded.
The blue fog wasn't added in to cover anything up though, it was apparently an artistic choice. I think it was to add to the somewhat painterly art style, but I'm not sure as only the quest designer talked about it.
