It literally just tells you how your performance was compared to what players with that champion usually achieve.
I don't think you can find a complete explanation somewhere because it's pretty complex.
It looks at your kills, deaths, assists, minion score, towers pushed, damage taken ... basically all your scores and what type of items you built, role and position you played and gives you something between S and D to tell you if your performance was weaker than the average or better and if so, by how much.
I can still remember when everybody got S-B's because the system had next to no data to compare.

It's a new system they gave us.
It gathers all your scores ingame, your champ and items, position and role and compares all these to the database.
