Go to Nexus mods website and download some mods to increase performance.
Serious answer without knowing your specs?
Go to your local computer place, ask them to install another 2gb of ram.
Or if you have the money, to swap out the graphics card for a newer one.
That's most likely the issue, i doubt your confident enough to simply to this yourself and order the parts online since your asking in the first place.

I know theirs a youtube video out there explaining why fallout is having issues on certain cpus. look up, "fallout 4 on 4690k review."
