I've heard every line of dialogue in the game time and again since I was 15 and the game came out on PS2. Nowhere in any of the radio (it's not a codec, they specifically mention this, "you fucking retard") calls does it say they removed the eye. In fact, Para-medic mentions that the eye is intact, just that it can't be repaired in survival viewer. He goes straight from the torture chamber to the cell, and straight from the cell to the waterfall where Eva gives him the eyepatch.
Furthermore, no Codec call in any Metal Gear game mentions "some doctor" removing the eye. He still has the eye in MGSV as well.

A lot of shit comes out of a gun barrel when you fire. If a firearm goes off right next to your eye, even if it isn't pointed at you at all, then a bunch of that shit (bits of metal, powder residue etc) is coming out of the barrel and going straight into your eye.
It is the reason why you can't point a black firing weapon directly at someone within a certain range - Although there is no bullet coming out, there is a bunch of other shit that isn't entirely pleasant.
Also blood effects in MGS have always been exaggerated,
