When a "normal" titan eats a human able to turn into a Titan (we call them Shifters) that Titan recovers its human body and mind and becomes a shifter.
We still don't know why some humans are shifters but we know a human can be transformed into a brainless titan using a serum or by "magic" that only Monkey Trouble knows.
