That's intended to be ambiguous. Miyazaki said he wanted players to have this constant feeling of "Is this all just a nightmare?" The ending (where you let yourself be killed in the Hunters Dream) leaves it open with your character "awakening" at morning time in Yharman. You're right that there is other dialogue that heavily implies the Hunter's Dream is a dream that all hunters visit for a time, Djura's dialogue also implies this and that he at one point may have dreamed of it.

It was his home town. He was a hunter who watched his fellow hunters turn on his home town when it was hit especially hard by the beat plague. He abandoned being a hunter and decided to guard Old Yharman from further hunts.

It's the night of the hunt. Anyone outside on a night of the hunt has a chance of being wrongly accused of being a beast. On this hunt the towns folk are losing themselves to the beast scourge. This is causing their minds to erode and they recognize little else but their old hunting compatriots, essentially becoming more like the pack of wolves they are turning into.

You're pretty on the money. The old college found Great One blood and remnants in the chalice dungeons. Laurence leaves the college and starts the church. The church gets everyone in town hooked on blood that contains what are essentially Great One bacteria. Said great one buggies cause the beats plague. On the night of the game more people are transforming all at once because the school of Mensis performed a ritual that made the Great Ones go crazy and be and in turn caused everyone in town to transform

That's for you to decide. From games always leave your character's previous life a mystery for you to explain.

A lot of this is open to interpretation. Perhaps secretly deep down, Gherman wants someone to finally take his place. Who knows what's a dream and what isn't, does it matter if everything is?

I'm not sure what you mean here but I think you mean the secret ending? Consuming enough of the umbilical cords is slowly changing you into a Great One, that's what causes the Moon Presence to attack you at the end, he realizes you're on his level and actually a threat to him. You're character is in fact in the beginning stages of being an ayylmao but not a tiny one like a Great One. It's implied the doll may possibly help facilitate this change and possibly even desired it to happen.

The basic timeline is:
- Alien Great Ones rule the earth
- An ancient civilization (the pale, rubbery guys) build the chalice dungeons, make contact with the Great Ones and get their shit wrecked.
- Much later, human scholars find the dungeons and the relics there, including alien blood samples.
- They found the various colleges and the healing church to study the aliens with the ultimate goal of human transcendence.
- Alien blood is used in medicine, starting the beast plague.
- Hunts are instituted to control the plague.
- The Moon Presence (final boss) takes control of the hunt using Gehrman as a proxy. Moon Presence seems to want to use hunters to kill rival Great Ones rather than just beasts.
- Everything goes to hell, you enter the plot.
Bloodborne is heavily Lovecraft inspired, so one of the main concepts is that the boundary between aliens and gods, science and magic is basically meaningless.

Bloodborne takes the Lovecraftian approach to dreams, in that they're separate realities that are crafted by particularly strong entities. Kinda like little pocket dimensions. You have the Nightmare of Mensis, who was crafted by Micolash in hopes to use a baby as a vessel for an alien god, and you have the Hunter's Dream, which is hosted by Gehrman, who gets his power from another Great One called the Moon Presence. If you fight Gehrma, you take his place as the host of the Dream. If you fight Moon Presence, you ascend and become a Great One yourself.

The monsters used to be his friends. Old Yarnham was one of the first places hit by the plague, and the Hunters responded by burning the entire place to the ground. Gatling sniper, went a little crazy at that point.

Your Hunter, for whatever reason, needs to get a blood infusion by a Yarnham Blood Magistrate. This taints him with Yarnham's corrupted blood, and draws him into the whole thing
