Depends on what the extra difficulty does. If it makes the enemies smarter and do new stuff I crank it up, if it just makes them damage sponges I keep it on normal or wherever combat doesn't feel like a chore.

Easiest setting for me.
I play games for their story, not gameplay.
Lately I always play the hardest, I never play a game two times so I prefer to have a nice challenge the only time I'll play it. Besides, if you start on hardest difficulty you get used to it and it doesn't seem that hard as the game advances.

In MGS games? Always the easiest. It breaks my immersion if the legendary snake gets caught by the very first guard because i'm too unskilled to play properly. also i care more about the story than the gameplay / difficulty.
i know you guys love to brag about your skill and everything, but who fucking cares. i'm not good in videogames and i don't want to be, i'm still able to enjoy playing them and i think theres much more important skills IRL than playing videogames.

Horror - Hardest
Action/adventure - Varies depending on the game. If it just adds tedium like Nier did then I'll play on easy or normal.
Platinum games - Normal and progress from there
MGS games - Easy and then the hardest
Yakuza games - Normal and then Ex Hard

For anything else, the hardest available.
I play Bethesda games on novice
Metal Gears on normal
Slashers on normal and then max scores on max difficulties
FPS on easy, I suck at this genre.
