He's updating things way too fast and making poor decisions because of it. It's not all immediately bad, but there are changes that do hurt the game like butt bombs healing the poop-based enemies, making one of the related challenges almost impossible for Afterbirth players to complete because of it.
Maybe in the future, all the issues would be fixed, but right now, nerfs to items and synergies are high priority for him for some reason.

There are better hellish items and even if it sucks there is a good chance to get a free guppy item.
You know what the game needs? An alternate route for those who have shitty items, the conditions to get that route is to ignore every devil and angel room items all the way to it lives, then you get a route which essentially like Floors II and III except you take extra damage like you would in Womb and every other after level, the final enemy would be something more like classic Isaac, and enemy with several forms with easy patterns.
The reward for beating it is an automatic Angel or devil item at the start of your next run that applies even if you quit the game, like an Eden Token.

I love the hell out of both Isaac games, but I really do wonder what the hell Edmund's thinking with some of the stuff we've seen so far with Afterbirth.

I got extremely lucky and managed to get a D4, and just rerolled my items until I managed to get Mom's Knife with the Holy Mantle.

In proper roguelikes the expectation is that you learn something about the game every time you die that will better equip you for your next run. Most of the time when you die in Isaac it's because you didn't get the right combination of items. Hell, it's not unusual toaborta run before you get to the womb because you know your damage output and survivability isn't up to snuff. Sure, with enough skill you can bridge the gap, but why bother when there's a potentially better loadout waiting on your next run? Playing a roguelike with that mindset, relying on the game to give you what you want rather than persevering through tougher parts, would result in very little overall progress. The problem with BoI is that that same mindset is a viable strategy for most players.
