It took me at least three playthroughs to find everything in NV. I still haven't seen every faction ending and only played through all the DLC once. I already kind of want to start a new playthrough just thinking about it.

Some of them are like that but there are also some interesting ones. One that I didn't notice until many playthroughs later is an NCR soldier whose husband got killed by a bunch of raiders. You can volonteer to go there and drag her husbands body away to a nearby NCR post so it can be buried properly. Not a great reward as far as I remember but it was a pretty unique quest
