Baldur's Gate 2 (1 to a lesser extent)
Planescape Torment
Daggerfall
Morrowind
Oblivion
Skyrim can be I guess
Ultima 7
Ultima Underworld
King of Dragon Pass
Stonekeep can be if you get past the rough nature of the game
Legend of Grimrock 1 and 2, especially with your own map on graph paper
If the Myst series count, then those
Heretic
Hexen (Tomb of Horrors is a great mod/mapset)
Stronghold HD
Dragon's Dogma if the pawns don't pull you out of it
Thief 1 and 2, 3 is the black sheep but I liked it
Witcher/Witcher 2
Sacrifice if you want a neat strategy game

Also Skyrim with sex mods.

The Witcher is easily the best when it comes to medieval settings and atmosphere.
Dark Souls is actually very good with it's world building.
Morrowind is fantastic, though not exactly "medieval".
Mount and Blade is absolutely great, especially with mods.
Gothic 1/2 are solid too. Fairly boring world, but well realized.
And I'm hoping that the Kingdom Come: Deliverance will stay true to it's name and delivers what they promised.
