Ages back I tried to make a new TF2 game mode with pizza delivery as it's theme.
Map was a simplistic city with random doors opening up. The team picks up their pizza box (a remodeled intel case) and fight their way to get there first.
Problem was keeping it fair. A door would always be closer to one base then the other, so one team might get a bunch of lucky rolls and win. Was messing with the idea of their bases being halves of the same building but then the conflict would be too central in one area.
I just shelved it.

While delivering the pizza, you get a short bio of the guy you're delivering to. From the bio, you have to be able to tell if he will tip you or not. Tampering with the pizza of tippers will get you fired. Not spitting into the pizza of non-tipping peasants will cause you to lose points.

Any of which happening either deducts from your score or ends the game.
Being raped causes a "big sausage pizza" QTE which results in a DDR style pressing of buttons you must complete in a set time and if failed, jizz falls out of your mouth onto the pizza, meaning it isn't accepted by the customer, or you take too long.
Also the QTE is from the delivery boy's POV with rendered pubes to look at that move realistically with jiggle physics.
You can also get a minigame where you fuck someone because they can't afford the pizza, but this means you deduct from your end score BUT get a temporary stat boost to try and recoup your earnings.
There is also a segment where you have to make the pizza, which can be done like the restaurant scene in Octodad, hell, make moving about like Octodad meets QWOP, and you have to move your feet and press them down on the pedals
Also some people offer you a drink when you deliver, meaning you can gradually get pissed up and your control over the character/vehicle suffers.
Just don't make it like postal.
