In the middle ages, you either had decent armor or no armor. Except for parts of the middle east and the roman empire, there wasn't any munitions plate until way later. People who expected fight would be wearing at the very least a maile shirt and the rich would be layering their armor. You have to remember only a small percentage of the population would be warriors (1-3%). They weren't serfs, they were free farmers, burghers, nobles, clergy, state-funded mercenary companies, etc. They had enough money for better than basic equipment, enough free time to train for war, and expected to make lots of money while on campaign. War was great for social advancement because you could literally make several lifetimes worth of peacetime income from ransoming prisoners and looting.
Leather was a very uncommon armor type in europe because the actual leather suitable for armor was a very small part of the animal. It's only with nomadic cultures, who lack large scale ironmaking, you see leather being the dominant armor type.

Until then, maile was the most common armor type in western europe, with plates often being layered over it for extra protection. By the late 15th century, solid plate armor became much more common. Knighthood was practically on death's door by that time, with most heavy cavalry being non-knight professional soldiers and foot soldiers rapidly replacing even these. So the whole idea of knights in full plate armor is kind of anachronistic.

Knights would. The rank and file sergeants and miles gregarius are there for a paycheck and have no opportunity for social climbing, for the most part. And it became common in the early renaissance period for wealthy countries and states to mass-produce armor for the use of their armies, in an attempt to give everyone a similar basic level of protection (munitions plate).
As noted in the thread, GoT takes place in a sort of late medieval, early renaissance era where it is entirely possible houses with the means are equipping their soldiers, or at least the cream of them, with a standardized equipment.
Also worth pointing out one of the thing that touched off the Wars of the Roses was over-mighty barons paying men to wear their colors (livery, liveried men) and fight for them.

The problem with that is that by the time full time professional armies became commonplace in europe, the amount of knights per men-at-arms had dropped significantly. Yet we still see knights being talked about like they're the shit in GoT and not a rapidly evaporating battlefield force.
Moreover, even pre-industrial professional armies had low levels of standardization. For most of the time roman legionaries existed, they were required to buy their own equipment (the cost may have been subsidized). The idea of everyone getting the same #11 sword is an industrial concept.

Late medieval armor was immune most stabbing and impaling attacks. That's part of the reason the last knights dropped their shield for polearms. There was also a greater emphasis on wrestling because it didn't matter how armor the guy opposite to you was wearing if you could break his neck.

Does anyone know the population of westerns? 40,000 fighting men seems kind of unlikely for an area that size, even one with a low population. I'm inclined to believe the north should be able to shit out at least 100,000 men if they have anywhere near 4 million people in population. Of course, they probably wouldn't deploy more than 20,000 in one place due to logistical constraints.

England had a population close to 1.5 million, maybe around 1066, but Harold was only able to call out a maximum of about 14k realistically. And those were young fighting men.
You have to rely on what your nobles and sergeants can realistically muster in that time, where they ride and if they keep an accurate tally on the number of men who are capable of fighting, and finding the right percentage to stay in villages to protect and farm without it hemorrhaging food and income.
