Tolkien probably saw himself more as a modern mythmaker than a novelist. And he was fantastic at that, what happened to Fantasy afterhis time wasn't his fault but he did inadvertently cause it. All of a sudden every fantasy is about the "Dark Lord" rising and the copy/paste hero's journey. The East and South are places of mystery and wonder and the whole thing is set in a pseudo-medieval secondary world where magic rules and technological advancement is halted.
Say whatever you want about GRRM's views on war or his portrayal of the pseudo-medeival society but at least he isn't copy/pasting Tolkien like every other High Fantasy author has been doing since the LOTR books came out.

GRRM is a good character writer, but is horrible at writing actual plots. He can make characters tick and resonate and can write good character arcs, but when it comes to actual storyline or world-building he is absolute shit at this.

He always relies on exponent to drive his books. MOAR characters to explain previous plot developments and characters. MOAR territories and factions to explain what's going on. MOAR macguffins to deal with existin ones. Problem is, the more you add the books, the more you have to deal with and since GRRM is bad at dealing wioth such things, it all has snowballed into such a mess that it's probably better to abandon the series altogether.

Your definitions of "hack" and "talentless hack" imply that the people on this list have at least enough talent to make one good movie/game/novel, but then repeat the same formular over and over.
David Cage never had any talent and never made a good game. Fahrenheit/Indigo Prophecy is easily one of the worst games ever made, and Heavy Rain was obviously done by a ghost-director but they put David Cage's name on it for some reason.
