The two entities have to be physically fighting. Sadako is operating on a time limit, she cant kill her victims until the seven days are up. Kayako has no time limit and can kill her victims whenever the fuck she feels like it. So I'm guessing Kayako will be spawning and trying to kill the victims throughout the seven days, prompting Sadako to spawn early and fight off Kayako until her victims survive till the end of the seven days, so she can claim their lives.
Then throughout all of this the victims are gonna be trying to find a way to stop Kayako. The Saeki curse has had a 100% fatality rate. I'm guessing the victims find out how to pass on the Sadako curse really early on, but decide not to pass it on because Sadako is the only thing keeping them alive from Kayako..

Ju-On: The Grudge is infinitely better than the American remake. First they made some made-for-TV movies, Ju-On: The Curse 1 & 2. Some studios saw the potential and made theatrical remakes of them as Ju-On: The Grudge 1 & 2. You might've watched JOTC, which isn't as good because the low budget hinders a lot of the scares and makes them laughable. JOTG is the best one. If you that was the one you watched and you still didn't like it, you probably have shit taste.

Just because you come in contact with an infected person doesn't mean you're 100% marked for death. Seems to depend on how long you spend around the infected person / if you're around the infected person when one of the ghosts attack. Otherwise brief interactions with the infected people seem okay.
