The saviors extort other communities for supplies under the pretense that they're keeping them safe from walkers, which they may have done since they have superior numbers and weapons. However, if someone doesn't fall in line with their demands, they kill them. Everyone that follows Negan either does it because they worship him, or they're terrified of him.
