On a net basis they instilled fear, but also respect. So a guy would pay them protection money from thieves and rival gangs. They'd slice some extra profits from the protection money, and if they needed to use the business for something under the tables they could pay the owner a little extra to do it. If they're paying properly, you don't fuck with them. If they stop, or are late, you scare them. If they try a second time or continue not paying, you break something to make them remember you for a while.
In addition, they have the ability to loan their capital for those who can't get a loan from a bank. Usually they show the gambler with a serious problem, which probably happens but is unrealistic. You'll usually have people who are simply don't have the credit to receive a loan. Still they're Italian or Irish or whatever, so they are allowed to receive a loan from their "family". That's how mobs work ethnically.
So basically it works for those ethnically similar, for their benefit and protection, by making capital and reinvesting it in the community. Sure there are tyrants, but respect from other Capos matter and if you step on too many toes you don't last long.
And yeah, you can kill one of them, but if they know that it was you, you're dead in the water. They don't stop until you're dead from an accident or even just a hit. It's a sign of disrespect to the Capo, and they got to keep others in line.
