To me it's really not so much motivating as comical.

I assumed that most parts were real (the sex stuff, the homeless man, the door men, and the inspector) and knew that the shit that he did later on and the real murders and the explosions and all were all in his head.
I think what fueled his inhibitions was some event that occurred early in his life and the lack of genuine affection directed towards him. Perhaps he was abused or used? He was only ever close to humanity when he was given genuine affection (refusing to kill his secretary and the gay guy).
