When I watch films set in New York, they usually have some seedy bar or greedy pizzeria. Couldn't you start the same business in a smaller city? There'll be less competition and the start-up/recurring costs will be cheaper.

This. Plenty of homeless shelters and food banks in my city but they're rarely taken advantage of by homeless because they require lots of personal information including SS which will pull up all their records including outstanding warrants and such and the homeless will be taken into custody.
Shelters are drug free zones as well so you can't be a textbook bum and live there, so they see it as just another jail cell.

Many homeless people are mentally ill. There is no place on the planet that treats the mentally ill with respect and dignity. In fact a lot of homeless people are war vets who have major psychological problems. It's difficult for them to break the cycle of homelessness let alone walk into a welfare office on the high street.
And to state the obvious, a bigger city does not equal better living conditions for the poor and people living in poverty. If anything it's compounded and worse in the bigger cities.
