there is only one thing that has ever been fake on the show, and it wasn't even fake, just replicated.
when the old man is telling him that he drinks his grandson's pee to live longer, Nathan apparently burst out laughing, as did the crew, and they had to stop for about five minutes. Nathan than just apologized to the old man, and told him to start telling the same story, since the last take was ruined by laughter.

The adherence to the classic 'set up', the 'stooge' etc is all very traditional vaudeville. It's 100% committing to the gag. Part of why it works so well is because Nathan is willing to put himself into situations he himself finds awkward and horrible, to give a natural reaction.
He is a comedian playing a character who is always trying to stay in another character in absurd situations he creates for himself to help motivate him to stay in character.
