The whole movie was about Ziegler trying to entice and initiate Harford into the sex cult. The film starts with Harford being seduced by two models who offer to take Harford "where the rainbow ends".
As his interest is piqued, he is quickly taken from the scene to help Ziegler cover up his infidelity with the overdosed Mandy. At the end of the scene Ziegler says to Bill: "I probably don't have to say this, but this is just between us".
Harford's wife asks him why Ziegler invites him to those parties every year, he responds 'for making house calls'.
A doctor is a powerful member of society and Bill uses his "power" as a doctor repeatedly in the film to access places normal people cannot.
